What information do we collect and how do we use it?
The information that the Midwest Center for Brain Health collects varies depending on the activity you are interacting with online. The following are examples of activities for which we will need your personal information: contact us forms, discovery call applications, online assessments, newsletters subscriptions, appointment scheduling, affiliated third-party applications and microsites, and products and services sales.
The Midwest Center for Brain Health also collects anonymous demographic and interest information using Google Analytics. This information is not personally identifying and may include your ZIP code, age, gender, preferences and interests. You can turn off Google Analytics’ tracking at any time by using the Google Analytics opt-out browser add-ons currently available.
If you have not opted out of receiving marketing materials, we may also use the information we collect to occasionally notify you about new services, research and news about our organization that we think you’ll find valuable. You can choose not to allow us to use your personal information for direct marketing purposes by indicating your preference at the time of collection or by “unsubscribing” to the promotional email in the link provided at the bottom of all correspondence.
What information do we share with or disclose to third parties?
The Midwest Center for Brain Health does not sell, trade, or rent your personal information to others. This does not include information provided to trusted third parties who help us in operating our website, conducting our business, and generating statistical analysis for use by Midwest Center for Brain Health.

We may use IP addresses to analyze trends, administer the Site, track user’s movement for aggregate use. IP addresses are not linked to personally identifiable information.
